Inkestry Tattoo Consent Form (ITCF)

This form MUST be completed prior to your appointment date and time.

COVID-19 Guidelines & Waiver (CGW) •

In exchange for being permitted to enter the premises of the Tattoo studio (the "Business") by entering (the "Premises") to receive artwork, tattoos/piercings (the "Activity"), I agree to the following as described in this contract. I will follow all of the instructions of the Business while on the Premises, including: Following the Membership Agreement, tattoo/piercing aftercare guidelines and all other studio policies.


I am aware of the highly contagious nature of COVID-19 and the risk that I may be exposed to or contract COVID-19 by being on the Premises and engaging in the Activity. I understand that I may be asked to temporarily remove my mask to complete the elected service procedure I requested to be provided by Mr. Mabee and I agree to comply when asked.


I acknowledge that I am voluntarily entering the Premises to engage in the Activity with knowledge of the danger involved. I hereby agree to accept and assume all risks of personal injury, illness, disability, or death related to COVID-19, arising from my being on the Premises or engaging in the Activity, whether caused by negligence of the Business or otherwise.

Tattoo Aftercare Instructions (TAI) •

The first few days and weeks are probably the most important for your new tattoo. Make sure you care for it properly. Below is a general recommendation, and not specific to each individual. Consult your Tattoo Artist and your Doctor if you feel you are having issues or complications.


1. Leave the bandage that you received after the tattoo on for 2 to 10 hours. Tattoos will ooze, this is a normal part of the healing process. The bandage is there to keep the airborne bacteria and any other contaminants in the environment from invading your wound.

2. Make sure your hands have been washed thoroughly before you remove the bandage.

3. After removing the bandage, cleanse the tattoo area with soap and water. Do not scrub the tattoo with a brush or towel (this should not be done until The Tattoo has healed). The idea behind the washing is to illuminate any dead cells they have been allowed to accumulate on the surface.

4. Dry the tattooed area by gently patting the area with a clean, sterile towel.

5. Let the tattoo dry completely (approximately 10 minutes).

6. Lightly rub on moisturizing hand lotion such as Curel.

7. You do not need to re-bandage the tattoo at any time during the healing process.

8. Repeat steps above for 3 to 4 days, approximately six times a day.


The idea behind this part of the process is to not let the Tattoo dry out. After a few days you may notice some peeling and possibly a little scabbing. Excess of scabbing can indicate a poorly done Tattoo, but some scabbing maybe normal. It is at this point in the healing process that or Tattoo will begin to Itcch. RESIST the urge to satisfy your desire to scratch, rub, or pick at The Tattoo.


Remember - the first few days and weeks are probably the most important for your new tattoo. Make sure you care for it properly. Consult a physician if you experience symptoms such as excessive elevated body temperature, swelling, redness, yellow or green discharge, fever, tenderness of the procedure site, purulent drainage from the procedure site, red streaks going from the procedure site toward the heart.


DO:

Stay out of the sun until your tattoo has healed completely.

Apply sunblock to Tattoo after it has healed whenever it is exposed to the sun to protect it from damaging ultraviolet rays.

Refrain from all recreational water activities until healing is complete.


DO NOT:

Swimming in pools, lakes or hottubs, No recreational water activities.

Use hydrogen peroxide or rubbing alcohol on your tattoo. Itch, Pick, or scratch the tattoo. 1. Participate in activities that will make you sweat. 2. Gardening. 3.Have contact with animals at the procedure site.

Medical Readiness Questionnaire (MRQ) •

I AM NOT pregnant or breast feeding,

I DO NOT have a history of herpes, hepatitis or any other illness/infection/viruses that are transferable upon contact or exposure to blood,

I DO NOT have herpes infection at the procedure site,

I AM NOT allergic to latex or antibiotics,

I DO NOT have diabetes,

I DO NOT regularly take blood pressure thinners, and I am not currently taking any antibiotics or other prescribed medication,

I DO NOT have hemophilia,

I DO NOT suffer from epilepsy,

I DO NOT have cardiac valve Disease,

I DO NOT suffer from any other bleeding disorder,

I DO NOT have a history of medication use and

I AM NOT currently using medication, including being prescribed antibiotics prior to dental or surgical procedures,

I AM AWARE that exposure to external pathogen such as other peoples bodily fluids puts me at risk for transmission of viral or bacterial infection,

I DO NOT have any other risk factors for blood-borne pathogen exposure,

I understand and agree that tattoos/piercings create a permanent change to my body.

Release & Waiver of Liability (RWL) •

The Client recognizes that there are hazards and risks connected with applying a tattoo/piercing and receiving a tattoo/piercing. These risks include, but are not limited to, needle stick, contaminated liquid splashed in eye/nose/mouth/clothes, uncontrolled urinating/defecating, exposure to blood borne pathogens, fainting, skin redness, swelling, tenderness of the procedure site, red streaks going from the procedure site towards the heart, elevated body temperature, or purulent drainage from the procedure site.


Client agrees to defend, indemnify and hold harmless Inkestry Custom Tattoos against any loss, damage or expense incurred by reason of any claim or liability based upon personal injury (including death) or property damage arising out of the negligent or intentional action of client. Client further agrees to release Inkestry Custom Tattoos and its owners, officers, agents, employees and/or affiliates from any and all liability arising out of injury to Client, and further agrees to defend, indemnify and hold Inkestry Custom Tattoos, studio owners, officers, employees and/or affiliates free and harmless from against the same.


Client acknowledges that surveillance cameras are in use for the protection of the Facility, its equipment, and its members. Client hereby consents to be photographed and/or recorded for such purposes.
